SPECIALIZED NEUROREHABILITATION SUPPORT AND EVALUATION COMMUNITY HEALTHCARE TEAM
This team is made by expert practitioners from Institut Guttmann, supported by the rest of the organization, whose main aim is to interact with other practitioners from the community healthcare networks to pass on to them the knowledge and skills necessary for caring for people with a neurological disorder, be it spinal injury, acquired brain damage, neurodegenerative diseases or, generally speaking, all other neurological diseases with a low incidence that tend to be highly disabling and not very well known by health practitioners.
Their objective is to contribute to adapt social resources, public and private, existing in Catalonia to the needs and assistencial peculiarities of this collective.
The team carries out the following tasks:

Easing off going back home and community reintegration for a person with a physical neurological disability. |

Managing cases by comprehensively evaluating these people’s needs and making a customized care plan for them. |

Expert advising other healthcare and social workers delivering bedside care. |
| · |
Contributing to the specialized training of practitioners in these areas and promoting specific research into community healthcare. |
